General Information

Name– Please provide any previous name(s) if you have changed your name.

Primary Phone – The number at which you would like us to contact you during the application process and for a final decision.

Addresses– Please provide your current addresses.

Education

You are asked to list the schools/colleges/universities you have attended with dates and whether a diploma/degree was received.

References

Reference forms will be emailed from ULS. Ensure reference email addresses are accurate. Provide their first and last names ONLY. Due to the Family Education Rights Privacy Act of 1974 (FERPA), the ULS Policy requires original references for your application.

Pastoral Reference – Must be from a pastor at your home church.

Background Check

All persons submitting applications for certificate programs to ULS will be required to complete Background Check Authorization provided securely by Castle Branch. An application will not be considered by the Program Directors until we receive the results. These results, along with all application materials, are kept confidential and included in the applicant’s file as a part of the application process.‍

Application fee: $50 (non-refundable)
Fall Application Deadline June 1st*
Spring Application Deadline November 1st*

To be considered for admission, your application must be completed, and the Admissions Office mustreceive your Pastoral Reference and the final Background Check report before the application deadline dates.  

 Once all your application materials are received, you will be notified by the Admissions Office that your file is complete.  Then your file will be reviewed by the Certificate Program Directors, you will be informed of the decision by email and/or a mailed letter.
